From 1 July 2025, contract cleaning and security employers must attach a leave nomination form(s) when submitting claims for reimbursement.

Each worker must complete a leave nomination form to confirm that they want to take traditional long service leave from their employer for a service period recorded with the Authority.

Use this form to authorise a person or organisation to act on your behalf when dealing with the Portable Long Service Authority.

A registered active employer who ceases to be an employer for a covered industry must give the registrar written notice within 14 days of so ceasing. Written notice must be in the approved Change of Status form.
